Facility Reporting
HOPDs and ASCs may report C9761 when the Axis II Clerixā„¢ is used during ureteral lithotripsy and the documentation supports the full code requirements, including use of a suction-integrated ureteroscope or steerable ureteral catheter with vacuum aspiration.
Axis II Clerixā„¢ Used With HOOVER FANS
When Axis II Clerixā„¢ and the HOOVER FANS suction-enabled ureteral access sheath are used during the same case, the facility reports C9761 as the procedure code. C9761 and C8014 should not be reported together on the same claim.
Device Reporting
- Medicare HOPD: Report C1747 for the single-use Clerix scope. Add C1889 when the HOOVER FANS sheath is also used. Device payment is packaged for 2026, but the device charge should still be reported.
- Medicare ASC: C1747 and C1889 are not required.
- Commercial payers: C1747, C1889, and revenue code 0278 may be required depending on payer policy and site of service.

The operative record should clearly identify:
- The Axis II Clerixā„¢ by its full device name
- The ureteroscopy, pyeloscopy, and lithotripsy services performed
- Use of the suction-integrated ureteroscope or steerable ureteral catheter
